Multi-Sector Service Accountability Agreement

The Middlesex Hospital Alliance (MHA) has an approved Multi-Sector Service Accountability Agreement (M-SAA) relevant to the Alliance’s Four Counties site. This posting is in compliance with a directive from the South West Local Health Integration Network (SW LHIN).

According to the Local Health System Integration Act, 2006, the service accountability agreement enables the LHIN to provide funding to the health services provider (HSP) for the provision of services. It supports a collaborative relationship between the LHIN and the HSP to improve the health of Ontarians through better access to high quality health services, to co-ordinate health care in local health systems, and to manage the health system at the local level effectively and efficiently.

The M-SAA document is produced from a “template” supplied to HSP by the Ministry of Health and Long-Term Care and includes the contractual performance targets. The programs and services being provided at Four Counties Health Services are the Adult Day Program and the Supportive Housing Program.
